- Dean confesses to Sam that their dad told Dean something about Sam -- look out for him, and save him; but if you can't you have to kill him. Sam is freaked out and heads out on his own for a while.
- Sam is the subject of a supernatural vision; the demon-touched psychic is a woman (Ava Wilson), the first female of the "children" that has been shown on the show. It turns out that her vision is of Sam dying in an explosion set by Gordon Walker, the hunter from 2x3 "Bloodlust". Gordon gets the drop on Dean and holds him hostage to draw Sam in.
- Talking to Ellen Harvelle, it seems that Jo has been missing since shortly after the last episode she was in (No Exit).
- Worried about Ava Wilson, Sam brings Dean to her house; they find her bloodily murdered fiance, and her ring; she is missing.
