- Apparently they've been looking for Ava (from prior episode "Hunted") for a month.
- They take a case involving a lot of mysterious deaths at an inn. The innkeeper and the porter each assume that Sam and Dean are a gay couple antiquing. "What'd you mean we looked the type?" Dean asks anxiously. ... "Of course the most troubling question is why do these people keep assuming we're gay?" he asks Sam later. "Well, you are kind of butch," Sam replies, "Probably think you're overcompensating." Dean laughs a little weakly.
- Sam gets drunk and makes Dean promise that he will fulfill John's orders and kill Sam if he turns evil.
- The mysterious deaths are caused by the ghost of the grandmother's twin, died in childhood; her Creole nanny taught Grandma (as a child) how to do some hoodoo protections but those began to lapse when Grandma had a stroke. The family is selling the hotel and planning to mvoe away, but Maggie (the ghost) wants to keep the kid there forever. Maggie tries to drown the little girl, but Grandma tells Maggie she'll stay there instead, and offers to sacrifices herself so that her daughter and granddaughter can go.
