- Dean is electrocuted by a supernatural entity (a "Rawhead") and his heart is terminally damaged, leaving him only a few weeks to live. Sam cuts a deal with a faith healer, but it turns out that it is the faith healer's wife who is really working the mojo -- using witchcraft to trade some people's lives to the Reaper for other people's lives. Dean is healed but at the cost of someone else's life. Another woman is not saved, while Dean keeps his life.
- Note - the faith healer's wife, Sue Ann, heals those whom she perceives as "righteous", and directs the Grim Repear towards those she perceives as unrighteous. These included an out gay teacher; an abortion rights activist; a critic of her faith; and eventually Dean (after he tries to stop her).
